Attention all Deacon Jones Memorial Tournament participants,

Due to health reasons, Ed Ezzell is unable to continue working this year's tournament. As most of you know, Ed has been the face of this tournament for many, many years so his presence is sorely missed.

Ed had a process that relied heavily on phone calls and his personal relationships with players and we're working to verify the roster for this year based on the information we got from him. Unfortunately, going forward we won't be able to dedicate someone to personally call everyone so we are asking players to provide us a valid email address so we may communicate important tournament information regarding event schedules, tee times, etc.

If you were contacted by Ed and committed to this year's tournament we ask that you please fill out the information below so we can verify your attendance and determine availability for others who are trying to sign up.
